This notice describes how protected health information about you may be used and disclosed, and how you can get access to that information.

What the notice covers
- How protected health information is used for treatment, payment and health care operations.
- Uses and disclosures that may be made without your written authorization, and those that require it.
- Special protections that apply to psychotherapy notes and to communication with other providers, including collaboration with parents, physicians or dietitians when appropriate and authorized.
- Your rights: to inspect and copy your record, to request amendments, to request restrictions, to request confidential communications, to receive an accounting of disclosures, and to receive a paper copy of this notice.
- How to raise a concern or file a complaint, and that you will not be retaliated against for doing so.
- The effective date of the notice and how changes to it will be communicated.
